Rules of Play
Purchaser must be 18 years or older to play.
Ticket buyers must be in the province of Ontario at the time of purchase.
Tickets will be sold using an electronic raffle system supplied by Canadian Bank Note Company, Limited.
Tickets can be purchased at 1750 Russell Road, Ottawa ON K1G 5Z6, through Perley Health Foundation and online at www.perleyhealth5050.ca.
Draw #1: Ticket Sales - Starting at 9:00 am on September 30, to October 31, 2022, until 11:00 am. Draw to take place at 1:00 pm on October 31.
Draw #2: Ticket sales start at 9:00 am November 1 to November 30, 2022, until 11:00 am. Draw to take place at 1:00 pm on November 30, 2022
Draw # 3: Ticket sales start at 9:00 am on December 1, 2022, to December 29, 2022 until 11:00 am. Draw to take place at 1:00 pm on December 29, 2022.
Draw #4: Ticket sales start at 9:00 am on January 3, 2023, to January 31, 2023, until 11:00 am. Draw to take place at 1:00 pm on January 31, 2023.
Draw #5: Ticket sales start at 9:00 am on February 1, 2023, to Feb 28, 2023, until 11:00 am. Draw to take place at 1:00 pm on Feb 28, 2023.
Draw #6: Ticket sales start at 9:00 am on March 1, 2023, to March 31, 2023, until 11:00 am. Draw to take place at 1:00 pm on March 31, 2023.
Tickets can be purchased for $10 for 20 numbers, $20 for 50 numbers, $40 for 150 numbers and $75 for 500 numbers.
Tickets can be purchased online or in person. Tickets can be purchased online at www.perleyhealth5050.ca with a credit card. Tickets can be purchased in person at Perley Health Foundation office located at 1750 Russell Road, Ottawa ON K1G 5Z6. Monday through Friday, inclusive from 9:00 a.m. to 4:00 p.m. Cash will not be accepted.
After buying, patrons will be given by the sellers, a receipt with their 50/50 registered numbers from a Random Selection System via email. No specific numbers can be given as numbers are generated randomly.
The winning ticket number will be selected by a Random Number Generation (RNG) system from all raffle numbers sold for the draw.
Perley Health Foundation, Staff, as well as their family members living in the same household are not permitted to purchase tickets and are not eligible to win.
One cash prize will be awarded for each draw, and the winning ticket will be posted on www.perleyhealth5050.ca. The holder of the winning ticket will be contacted subsequent to the draw.
The winning ticket holder has three months in which to claim their prize from the date of the draw their ticket was purchased for.
The winner must verify their identity by supplying two pieces of identification that includes their name and address. One piece of identification must be government issued and include a photo and date of birth.
Once the certified winner is verified, and provides identification with photo ID, they will be given a cheque in the amount of the declared prize which is, at minimum, 50% of the total sales for that game. If the winner cannot provide photo ID, then the winning proceeds will be held until such identification can be provided.
Cheques will be issued by Perley Health Foundation and may take up to 6 weeks to receive your winnings.
In the event that no winner comes forward, the winning number will be stored in a safe and held for three months. After that time period has elapsed the prize will be donated to Perley Health Foundation with the approval of the Alcohol and Gaming Commission of Ontario.

Any transaction cancellations initiated by a party other than Perley Health Foundation will result in the associated tickets being considered void, and as a result, not eligible to win the prize.
Tickets cannot be transferred or resold to another party.
